Terms of Use

These terms govern use of credentixhealth.com and the Credentix Health application (“Service”) provided by Credentix Health (“we”, “us”). By creating an account, inviting users, or submitting the request form, you agree to these terms and our Privacy Policy.
The Service
Credentix Health is software for clinics, medical groups, health systems, and credentialing agencies to organize provider professional information, documents, and enrollment workflows. We do not provide patient care, make credentialing or privileging decisions, or act as a medical staff office. Those decisions stay with your organization.
The Service is for professional credentialing operations. Do not upload patient medical records.
Accounts and your organization
- You must be authorized to act for the organization whose account you open or administer.
- Your organization controls who is invited, what they can see, and the accuracy of provider files you store.
- Keep passwords confidential. Tell us promptly if you believe an account is compromised.
- You are responsible for activity under your organization's users, including agency staff working on a client clinic.
Acceptable use
You may not:
- use the Service for anyone other than your organization and its authorized providers
- attempt to access another organization's data
- probe, overload, or disrupt the Service except through documented support channels
- upload malware or content you do not have the right to store
- resell the Service without a written agreement with us
Fees
Paid plans are billed through our payment processor. Fees, limits, and the current plan appear at checkout or on your billing page. Subscriptions renew until you cancel. Taxes may apply. We may suspend access for unpaid invoices after notice.
Availability and support
We aim to keep the Service available and will try to give notice of planned maintenance. We do not guarantee uninterrupted access. Support is available at [email protected].
Data and privacy
Your organization owns the provider records and documents it enters. We process that information to run the Service, as described in the Privacy Policy. You must have a lawful basis to store information about the people in your roster.
Intellectual property
We and our licensors own the Service, branding, and software. You keep ownership of content you upload. You grant us a limited license to host and display that content as needed to provide the Service to you.
Disclaimer
The Service is provided “as is.” We do not warrant that it will meet a particular accreditation outcome or payer deadline. To the extent allowed by law, we are not liable for lost profits, lost data, or indirect damages, and our total liability for a claim is limited to the fees you paid us for the Service in the three months before the claim.
Term and termination
You may stop using the Service and cancel a paid plan from billing or by emailing us. We may suspend or close an account that violates these terms, fails to pay, or creates a security risk. After termination we may delete or return data as described in the Privacy Policy and any written order.
Changes
We may update these terms. The effective date above is the current version. Material changes will be posted on this page. Continued use after that date means you accept the update.
